LEGACY LAND CONSERVATION PROGRAM GRANT APPLICATION CYCLE FOR FISCAL YEAR 2025 (APPLICATION PERIOD HAS CLOSED)

PLEASE REGISTER BY SENDING AN EMAIL TO: [email protected] WITH EMAIL SUBJECT HEADER THAT READS LLCP_FY2025_LAND ACQUISITION_<APPLICATION TITLE>
LLCP WILL THEN SEND BACK AN EMAIL THAT EXPLAINS HOW TO ACCESS THE ONEDRIVE FOLDER.
NOTE: Include Maps and Photos with Form 5 Package. See FY2025 Application Guidelines Document.
NOTE: Some applicants may seek to acquire multiple parcels, but aren’t sure at this time which ones will be presented in the final application. Additional information about each parcel that might be acquired should be included in the following form:
STEP 1a: Complete Forms 5, 5a, 5b, and 5c (Form 5 Series, download the fillable pdf files via the hyperlinks below)
Use computer to complete a fillable pdf form, or print a blank form and write-in the content.
STEP 1b: Save the completed forms using this filename format: FY2025_Land Conservation Fund_Form <number or letter of form goes here>_<short version of applicant name goes here>_<short version of application title goes here>
Example: FY2025_Land Conservation Fund_Form 5c_Kauai County_Good Kine Land.
For an agency, county, or organization that submits more the one application, please use different application titles that are clearly distinct from each other.
For an application that was/will be submitted to other funding sources, please use a consistent application title across all application platforms.
For a fillable form completed on a computer, use “Print to PDF” to make the application file. This method will deactivate the fillable functions in the form, cement the filled content, and avoid future problems with processing the form.
For a printed blank form with write-in content, use “Scan to PDF” to make the application file.

Form 5– Agency Consultation and Request for Advice: Information from Grant Applicant
Form 5a – Agency Consultation: Department of Land and Natural Resources
Form 5b – Agency Consultation: Department of Agriculture
Form 5c – Agency Consultation: Agribusiness Development Corporation
- Form 5 Guidance (contact the Program Office for assistance with special situations, such as more than five parcels, portion of parcel, or horizontal property regime)

STEP 1c: In a single email to [email protected], attach each completed form as a separate file (four attachments). Use this format in the subject line of the email header:
FY2025_Land Conservation Fund_Form 5 Series_Kauai County_Good Kine Land
Submit the completed Form 5 Series to [email protected] by August 23, 2024 to allow for adequate agency response time.
Later in the process, an applicant will receive Forms 5a, 5b, and 5c, as completed by the consulting agencies, and will upload Form 5, and portions of Forms 5a, 5b, and 5c, into the final online application.
STEP 2: Complete and save Forms 1, 2, 3, 4, 6, and 7 as in Steps 1a and 1b above.

Form 3 – Land Acquisition Purposes
Form 4 – Stewardship and Management
Form 6 – Importance and Threats
Form 7 – Estimated Funding
STEP 3: Complete the Form 8 and 9 financial/accounting worksheets in the Excel workbook provided below, then save each worksheet as a separate *.xls file, then “Save to PDF” for each *.xls file, using the same filename format as above.
OR complete Form 8 and Form 9 in pdf form by editing the blank *.pdf file with Adobe tools, or by printing the blank form, writing in the content, then “Scan to PDF.”

STEP 4: Using a different email address for each application, sign in to the online ONEDRIVE application portal (instructions forthcoming) and complete the online application, which includes completion of additional information and uploading the forms saved in STEPS 1, 2, and 3 above.
